This platform collects sensitive personal experiences. We limit what we ask for, act only on the permissions you select, and never publish anything automatically.

Information we collect

  • Contact details you provide: name, email address, optional phone number, and preferred contact method.
  • General location: state, county, and ZIP code.
  • Non-diagnostic descriptions of the patient's needs, age range, and the type of care sought.
  • Access barriers encountered, approximate wait times, and any narrative you choose to write.
  • The publication and contact permissions you select.
  • Provider applications, resource corrections, and contact messages.

What we ask you not to send

Please do not include medical records, clinical notes, insurance or Medicaid identification numbers, Social Security numbers, or full dates of birth. If sensitive identifiers are received, reviewers remove them.

How we use your information

  • To understand and document patterns of healthcare access barriers.
  • To produce de-identified aggregate counts for reports and policymaker briefings.
  • To publish a story only if you granted that specific permission.
  • To contact you only if you granted contact permission or wrote to us directly.
  • To verify and maintain the accuracy of the resource directory.

Consent and control

Every publication permission begins unchecked. Reviewers cannot publish beyond the permissions selected on your submission. You may withdraw a permission at any time by contacting us with your submission number.

Storage and security

Submissions are stored in a managed cloud database with row-level security. Only reviewed staff accounts with an assigned role can read submissions. Published stories are stored separately from the original submission and contact details.

Sharing

We do not sell personal information. We do not share identifiable submissions with providers, insurers, or government agencies. Aggregate, de-identified counts may be shared publicly and with policymakers.

Retention and deletion

Submissions are retained while they remain useful for advocacy and research. You may request a copy, a correction, or deletion of your submission at any time using your submission number.

Children's information

Information about a minor should only be submitted by a parent, legal guardian, or authorised caregiver, and should be limited to what is needed to describe the access barrier.
